Community Health Workers

Southern Jersey Family Medical Centers, Inc. is looking for candidates with talent, professionalism and compassion. We are currently seeking Community Health Workers to improve health outcomes in Atlantic County and Burlington County.

Summary: The Community Health Worker will work closely with the PACT Improving Pregnancy Outcomes (IPO) program, and will go out into the community to educate and work with specific target populations. Ideal candidate should have prior experience or education in health and/or social services. Local travel is required.

Hours: This is a full-time position that requires the ability to work a flexible schedule.

Position Responsibilities/Duties:

  • Communicate the purposes and services of the program to the target population
  • Use the Central Intake Process to refer at-risk individuals to the program for services
  • Establish positive, supportive relationships with participants
  • Effectively work with individuals from diverse backgrounds in reducing cultural and socioeconomic barriers between clients and institutions
  • Encourage patients to be active, engaged participants in their heath
  • Conduct risk assessments through the use of a standard assessment tool
  • Work collaboratively and effectively as a member of a team
  • Continuously expand knowledge and understanding of community resources, services and programs provided
  • Serve as a member on a local community advisory board
  • Other duties as assigned

Experience and Skills:

  • High school diploma or equivalent
  • Health/social services experience or education
  • Excellent verbal and written communication skills
  • Upon hire, must successfully complete 2-week training session for Community Health Workers as provided by New Jersey Department of Health.
  • Computer skills: EHR experience preferred
  • Valid driver’s license and reliable transportation
  • Bilingual preferred
