Seasonal Outreach Specialist - Migrant Health

Southern Jersey Family Medical Centers, Inc. is looking for a seasonal Outreach Specialist with the highest level of professionalism to step into our culturally diverse and fast-paced facility!

Temporary positions are available April-September!

Summary: The seasonal Outreach Specialist promotes preventative health by conducting extensive and varied health education activities in labor camps and other areas with a high concentration of migrant and seasonal workers. The seasonal Outreach Specialist also provides other services to migrant and seasonal workers, including but not limited to interpretation, transportation, case management, and assistance with medication and diagnostic exams. The seasonal Outreach Specialist will provide transportation to and from specialist appointments, hospital tests and the pharmacy.

Hours: Full-time and Part-time positions available! Hours may vary, but are generally afternoons, evenings and weekends.

Position Responsibilities/Duties:

  • Make periodic visits to labor camps/campsites located in SJFMC’s areas of service, identifying health problems and concerns of migrant and seasonal workers.
  • Conduct individual health assessments including health history, blood pressure, blood sugar screenings and referrals.
  • Perform group health education sessions and health promotion activities at the farm sites, community gathering points and health centers, as needed.
  • Maintain positive relationships with referring community resources/ health facilities and provides appropriate referral to health center and other community resources.
  • Collects, reviews, hands in and files SJFMC van maintenance documentation and mileage logs.
  • Document and inform Outreach Manager or other Administrative Staff about any incidents, accidents or problems pertaining to the Outreach Program.
  • Provide or arrange transportation as needed to/for farmworkers needing assistance getting to community organizations and health center.
  • Obtain transportation schedule from Manager or EHS, verify address/directions of new transportation patients.
  • Maintain mileage and maintenance logs for company vehicles, ensure SJFMC vehicles are wel maintained and well fueled at all times.
  • Document all encounters and activities in EHS and provide weekly progress reports from these encounters and activities.
  • Employ the highest standards of professional thinking, bearing, conduct and appearance.
  • Establish and maintain satisfactory relationships with personnel in all operating departments of the company, patients, doctors and the business/medical community at large.
  • Create and promote the company image through the exercising of sound, ethical business practices as related to patients, doctors and the business/medical community at large.
  • Perform other tasks as assigned by Manager or Coordinator.

Experience and Skills:

Position Requirements:

  • Bi-Lingual in Spanish or Creole
  • Strong computer skills
  • High school diploma or equivalent
  • Familiar with the community and farms within the service area preferred
  • Case management and Outreach experience preferred
  • Excellent verbal and written communication skills with focus on customer service
  • Valid driver’s license with good driving record
