Job Description:
POSITION SUMMARY: The Behavioral Health
Care Coordinator for Behavioral Health Services is responsible for
screening, prompt referral implementation, screenings and support of
Behavioral Health Services at the Southern Jersey Family Medical
Centers. The Behavioral Health Care Coordinator will monitor
utilization, track compliance with follow-up visits, assist with
resolving barriers in the use of outside agencies.
Hours: This is a full-time position that requires
the ability to work a flexible schedule. General daytime hours
Monday-Friday with at least one evening per week.
DUTIES AND RESPONSIBILITIES:
- Listen and provide emotional support and encouragement to
potential Behavior health patients, while utilizing evidenced-based
tools for screening and providing referrals

Ensure that patients understand screening tools and assist with
survey administration if needed

Will schedule appointments, make follow-up calls and assist with
community resource relations

Documents interactions and interventions in accordance with SJFMC
guidelines and ethics policies and procedures

Function as a part of a multidisciplinary team utilizing
assessment tools and surveys for patients ages 12 and over

Communicate effectively and interact will all levels of staff,
primary care providers, patients and community workers

Interact with patients in a courteous and culturally appropriate manner

Ensure the proper safeguarding of confidential program and
patient information, and comply with all Federal and State guidelines

Perform other duties as assigned to meet program or
organizational objectives
