Women's Health Coordinator

Southern Jersey Family Medical Centers, Inc. is looking for a talented Women’s Health Coordinator to step into our culturally diverse and fast-paced Hammonton, NJ facility!

Summary: The basic function of the Senior Nurse Manager of Women's Health is to strengthen SJFMC's footprint in the Atlantic, Salem and Burlington communities and to facilitate a larger impact in the coordination of the Women and Children that SJFMC serves. This position will partner with the Medical Director of OB/GYN and the AVP of Clinical Support Services and Community Engagement; the Senior Nurse Manager of Women's Health will work in alliance to enhance new and existing practices and initiatives, to further the missions, goals and strategic plans for the women’s health care coordination program. This includes, but not limited to, an emerging collaboration with external entities to heighten our visibility.  

Hours: This is a full-time position that requires the ability to work a flexible schedule. General daytime hours Monday - Friday with 1 to 2 evenings per week.

Position Responsibilities/Duties:

  • Oversees the Women's Health team 
  • Develops processes and protocols to maintain compliance of all initiative standards 
  • Orients obstetrical patients upon the first registration visit regarding the content and process of comprehensive maternity care services
  • Oversees processes and assists with patient services and linkages with appropriate agencies
  • Develops an individual care plan for each patient
  • Implements case conferences when necessary, with other care providers, to implement the care plan
  • Performs all record keeping duties related to maternity care services and chronic case management services
  • Provides, coordinates, reinforces and supports all health teachings with patients as needed
  • Obtains all information regarding labor, delivery and post-partum records within two weeks of delivery and coordinates all post-partum visits
  • Provides coverage to all OB patients as needed
  • Perform other tasks as assigned by the OB/GYN Medical Director, the AVP of Clinical Support Services and Community Engagement and/or the President/CEO


Experience and Skills:

    • Bachelor’s degree in Nursing required (Master’s preferred)
    • Registered Nurse Active New Jersey License
    • At least five (5) years of healthcare experience, preferably in the ambulatory setting
    • At least two (2) years of supervisory/management experience, preferably in the ambulatory setting
    • At least two (2) years of Women’s Health experience, preferably in the ambulatory setting
    • Strong operational mindset
    • Inspirational and Transformational leadership style
    • Able to foster positive relationships across all levels  
    • Full-time, flexible schedule
    • Travel required
