Case Manager - Refugee Program
Southern Jersey Family Medical Centers, Inc. is looking for talented
a Case Manager to step into our culturally diverse and fast-paced
facility and work with our Refugee Program
Summary: The Refugee Services program at
Southern Jersey Family Medical aids refugees in beginning their
lives in the United States by completing a set of core healthcare
services with clients and their families. The Coordinator of Refugee
Services is responsible for managing client caseloads and
relationships with program partners at other agencies.
The Coordinator of Refugee Services is expected to orient new
refugee clients, coordinate the delivery of core healthcare
services and screenings, supplemental services, and enable
clients’ self-sufficiency. General duties include but not limited
to the following:
Hours: This is a full-time position that requires the
ability to work a flexible schedule. General daytime hours Monday –
Friday with and at least one late night per week.
Position Responsibilities/Duties Include But Not Limited To:

Network with outside partners to provide services and create
opportunities for clients;

Maintain client case files and documents for each client.

Complete reporting requirements, as required per contractual agreement.

Report work schedule and overall trends in service delivery to
Program Manager.

Participate in department meetings and collaborate with program
staff and partners.

Attend required trainings and/or education.

Coordinate, in conjunction with resettlement program agency
staff, the schedule and delivery of services for each client.

Coordinate, in conjunction with resettlement program agency
staff, for cases that will be arriving to complete pre-arrival
services such as case file setup, and scheduling of preliminary documentation.

Coordinate, in conjunction with our Program Manager RN of Refugee
Services, the delivery of the following core services for each client:

Orientation and patient care plan;

Domestic Health Assessment and follow-up medical case management;

Assist in screening, enrolling and orienting clients to the
program services of Southern Jersey Family Medical Centers
