Experience and Skills:
• 1 - 2 years of experience working within a primary or ambulatory
care setting
• LPN, Social Worker, RMA or CMA preferred
• Case management
experience preferred
• Excellent verbal and written communication
skills and organizational skills
• Ability to educate, motivate
and inspire
• Valid driver’s license
• Bi-Lingual preferred
(Spanish or Portuguese)
• Computer skills
• High school diploma or equivalent
