Southern Jersey Family Medical Centers, Inc. is seeking a talented
Patient Navigators to step into our culturally diverse and fast-paced facility!

Track and monitor provider orders for outpatient services to
include laboratory, radiology, diagnostic testing and consultation services.

Create and attach orders for outpatient services requested by specialists.

Ensure appropriate medical records are available at scheduled appointments.

Contact patients and/or specialist as needed for missing reports.

Update demographics pertaining to address and telephone number as needed.

Provide patients with existing copies of lost or misplaced orders
upon request.

Facilitate the process for follow up appointments as needed.
-
Notify clinical staff of patient non-compliance.

Work to improve patient compliance through telephone calls,
written correspondence and the patient portal.

Support clinical staff to reduce gaps in care.

Assist with compliance projects upon request.

Routinely review provider patient panels supplied by the various
insurance plans to identify patients who have not presented for services.

Assist patients and families in navigating the complex healthcare system.

Attend all mandatory meetings and trainings.

Other duties as assigned or as needed to support organizational goals
